If you would like to make a request for a donation from the Winston-Salem Warthogs, please print and fill out the form below.  The form must be completed at least 2 weeks prior to the appearance or donation deadline.  You can also stop by the Warthogs office to fill out the form.

If you would like to request an appearance from Wally the Warthogs, please print and submit the form below.  The form must be completed and submitted at least 2 weeks prior to the requested date for Wally the Warthogs to appear.
